Quality Improvement

This year the surgery is undertaking a Quality Improvement Project for the Welsh Government looking into CKD. Kidney disease is a term used by Doctors to include any abnormality of the kidneys, even if there is only a slight change in function. ‘Chronic’ means a condition that does not get completely better and lasts over a long time. ‘Chronic’ does not mean severe.

Generally, CKD isn’t something that gets talked about much but is monitored in the background. CKD is more common than you think. It is monitored with blood and urine tests as part of annual reviews and is linked to other ‘chronic’ diseases.

Our project will be looking at undertaking a medication review for patients with more advanced CKD and offering additional medication as appropriate.  To compliment this, we will also be ensuring that our patients with Type 2 Diabetes and Essential Hypertension (High blood pressure) have an annual urine test along with their annual blood test.

We are undertaking this work alongside our existing annual review system. This will help us identify the ‘at risk’ group who need further input in terms of medication and monitoring.
